In order to provide exceptional comprehensive care, we ask that you download the New Patient Information forms that are located in the Downloads area. Please complete them before your scheduled appointment and bring them with you on the day of your exam, or Fax them ahead of time to 949-642-4338.
It is helpful if you prepare a list of current medications, including eye drops and supplements and bring this with you.
Before your appointment, find out if you will be given drops to dilate your eyes. Although the effects of dilating drops are temporary, you may need to arrange for someone else to drive or to pick you up after your exam. If you are scheduled for a dilated exam, please expect to be in the office for one and a half to two hours.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Please contact our office to find out which insurance companies we accept.
